Appearance: Naoto dresses, moves, and talks like a boy. While she no longer actively denies or tries to hide any clues to her sex (aside from continuing to bind her breasts), she doesn't go around advertising it either. If asked, she will answer that she is a woman but if someone just assumes she's a boy she may or may not correct them. It will depend on whether she cares about them enough to bother with the necessary explanations and questions.
Perceptive people will be able to tell she's a woman via the following characteristics:
Stature: Naoto is tiny
Face and skin: her jawline is feminine and she has, according to the girls "silky soft skin"
No Adam's apple
Hands: Her palms are slender and she has long, graceful fingers (but her nails are cut super-short)
The player has a personal preference for her Japanese VA (Paku Romi. A sample of Naoto's voice is about 1:45 in, here. Spoilers if you speak Japanese.) and so considers that to be Naoto's vocal quality. So Naoto's voice is low for a woman's even when she isn't trying to lower it, but not outside the possible range for a woman. Naoto is unlikely to actively lower her voice except in certain situations.
