This seems entirely plausible to me. I always wondered how much Sam spilled to the doc, cause he did look guilty when he came out. They were probably raised to bottle up both emotional and physical pain. Sam had Jess for a while, so he probably put aside that grin and bear it attitude. Sam wants to talk about things. I wonder if he was that expressive as a kid, or was it Jess' influence that brought out the "chic flic" emo spilling. I'm sure Sam felt a sense of betrayal towards both talking about Dean, and leaving Dean in the first place. Those boys are truly screwed up.

I suspect Sam as a little boy felt free to talk about what he felt and thought - he was well-cared-for, and probably felt secure for the most part - and I think that prompts kids to speak their minds, in hopes of getting more of what they want. Dean, on the other hand, was told from early on that taking care of Sam was the Prime Directive - that they had to be careful about what they said, and to whom. So I can see Sam feeling very free to talk up to a certain point (teen years, when John started shutting him down every time he objected to The Job), while Dean never was.
