I finally got to see this episode on Friday thanks to the NBA playoffs :P. I found it unsettling - although I sympathize with Castiel (he would gladly risk feeling bad, if only he could taste his PB&J) and adore the fact that he has learned that all human life as precious "even pig-headed Winchesters."
Caine annoyed me only because in my head canon Caine is Christopher Kane; leader of the Wild Hunt. I did like the First Sword; the glimpse I had of it made it look like the jawbone of a horse - or maybe a donkey. (as in Sampson - hmm. . .)
I already dislike next week's episode; Garth a werewolf? If they kill him I will be angry.
LOVED this episode. Best in a long time, I thought (even with the rewriting of Cain & Abel, which was at least interesting and cool to match to the boys).

I also find it interesting that Crowley is apparently taking orders from Dean when it comes to finding the blade. Hm . . .
