Yikes! Sorry you're in such a pickle with procrastination. Would making a schedule for yourself help? Such as, scheduling an hour to work on class/homework, then half an hour of doing something you like, followed by half an hour of cleaning...lather, rinse, repeat. I don't know, but I hope you sort it out so you're not unhappy any longer.
It sounds to me like you are a perfectionist and are sabotaging your school work because you can't be perfect. If you procrastinate enough and fail, it's because you put it off, not because you weren't up to the task of being smart enough to pass - Which is baloney because you are smart enough.
Also the sleeping/dirt bomb apartment sounds like you might be depressed?
*Sending love to you* Would writing a list of little things to clean around the house help? You could try to tick one off a day. I know how overwhelming stuff can be when you're not feeling your best.
