Why do "Have you ever been convicted of a felony?" and "Have you ever sat on a Jury?" share the same checkbox? It's like asking someone "Have you ever ingested sugar or PCP?"

On "2. DEMOGRAPHIC QUESTIONNAIRE," there is a series of yes/no questions on the bottom. "Have you ever been convicted of a felony? Have you ever sat on a Jury?" is set up as a single question as opposed to being 2 distinct questions.

As far as that last bit, it's a joke from departed stand-up comic Mitch Hedberg, complaining about the weird physical they gave him before a show, including two disjoint subjects being shoehorned into a single question.
