There haven't been any other solutions posted, and no one has proposed solutions through direct messages, so it seems like it's still unresolved. I haven't seen a lot of spam lately, however.
I suspect that when you are not seeing anything in the list of "xx suspicious comments" most of those are actually LJ heart-likes. If you click "xx suspicious comments" it will show you any actual posted comment that you need to unscreen or mark as spam. If it shows you nothing at all in the list, that number shown is more than likely just the likes for the post that came from deleted or banned journals, or suspect IPs the system locked out, or unlogged in people.
If you enable "show likes in comments" under Display in settings it will post a actual comment with a heart image every time someone likes a post. You can unscreen and delete them if you want, and I suspect it will no longer show the "xx suspicious comments" line. It won't, of course, remove that for older posts with existing likes. If you uncheck that option in display later and someone likes a post after that, it might once again begin showing you "xx suspicious comments" with no way to deal with it. LJ might have fixed that flaw, but I have information on that.
