My OTTB gelding is turned out 8 to 9 hours a day with a round bale and is worked five days a week for 30-45 minutes, with jumping once a week, 2'9" max. Feed and daily routine has not changed

I'm guessing you already have since he's being ridden regularly, but we also see horses stock up when they have problems with fungus, especially the boys and their back legs. Checking for scabs on his back legs would be another suggestion. Since he stands at his round bale in turnout, he's not getting the time to move around and graze like he should - that constant movement is what helps reduce the swelling. If possible, 10-15 minutes of handwalking a day would help. If not possible, spend more time warming up and cooling out when you ride, especially at the end of a ride. That extra time walking makes a huge difference.
I'd second extra walking; I used to do that with Feronia when she'd occasionally stock up, and 15 minutes of walking under saddle was enough to get everything moving again. (In her case, it seems to be caused by certain foods, like flax seed and a weed colloquially called "plantain" that grows in the grass at her barn.)
