You know, it's funny you asked this, because this is the only "pick one book" question I've ever been able to answer.
For me, the book is A Ring of Endless Light, by Madeline L'Engle. Broadly, it's a story about how the narrator, Vicky, deals with the understanding and reality of death around her, about life and who is a healthy person to love, and about dolphins and friendships.
None of the specific experiences mirror mine, and yet the way she views life and relates to people hits me every time I re-read it as being straight from my own heart.
Okay, so all Madeline L'Engle books have that bit to it, because Madeline L'Engle seriously seemed to have a direct line to my innermost me, but Ring is especially so for some reason.
