I could see getting the last one wrong, if you were careless and you assumed they were sloppy. If taxes equal government spending, then: A. government debt is zero is sort of right, if you think of it as meaning the debt incurred.
The only other one I could think of is the source for “wall of separation” - it's pretty easily narrowed down to Washington or Jefferson, but my only basis for choosing Jefferson was that I don't know Washington's views on the topic, while I happen to know that Jefferson's were consistent with that statement. Without that knowledge, it would have been a coinflip.
