1. Some people might be using a built in blog feature with their photo hosting site, like flickr.com. You can post a photo to LJ via flickr and it formats it for you (blogger does it too). Or you can learn HTML and do it manually, or type your entry up in something like FrontPage and fiddle with the formatting, then copy and paste that as your entry (I think that would work. I've never done it).
2. I think the layout scheme you use determines if there is a calendar. I didn't do anything to mine to get it. It's just there.
3. I don't think you can have a different icon appear with each new post you make. However if you friend yourself, when you view your friends page, you will see the icon that you chose for that entry.

As someone else said, it's part of the layout scheme you choose. I know, for instance, that metawidget's layout allows you to see each of the icons.
You know, most of this is probably answered in the FAQ, which is probably more helpful than what I told you. LJ has a built in photo thingy too, which I never use.
2 & 3 are options in the S2 style. Right now you're still using S1 (at least that's the way it seems when I look at your page)
