[Thrall is carrying a large wooden totem on his back.  Depending on when and where you run into him, this may involve him hiking through the forest with it or just carrying it through the village.

Why is he doing this, you ask?  ... Shaman stuff.  You're free to ask for a better explanation, of course...]

So... what do you get for putting up totems for the 'spirits'?

[Yeah, all orcs are over six feet. Thrall's probably close to seven.]

It's more that the totems are a thank you for what they do for us. They give us their power, and in turn we honor them.
