The first time I saw it as Cass was in one of the novelizations. It makes no sense that it's Cass since he only has one 's' in Castiel. I've never seen it as anything but Cas in fic.

To answer your questions...
1) It's definitely Cas. I've seen it written both ways, but Cass just doesn't make any sense to me- Cass is short for Cassandra/Cassie, not Castiel ^^
2) I imagined the guy who slipped Cas the drug as simply someone who wanted to... well, get into his pants LOL

the subtitles on the show have it as Cass, and I've seen it both ways in fanfic, though I do tend to think of Cass as short for Cassandra.

The man in the strangely fitting pants: random guy unless you really want to expand the story beyond the ending here and go somewhere a little darker & angsty.

This is personal preference only, the Castiel POV shouldn't parrot back the exact same dialog. You have lots of room to expand on the Dean & Castiel conversations where Sam wasn't in the room. Also lots of places to get into the 'moment', detailed narrative of Castiel experiencing the joy /end sarcasm/ that is Sin City.

Castiel getting ruffied at random is an interesting concept, I'd like to know why it affected him, when he had to drink a liquor store to get drunk!
