1) I'm going for around TIA time. Maybe on their way to America, or somewhere in there.
1a) I think from TDW. He locked Amy (and Rory) in the Tardis and didnt want them around. I think from this point the Doctor dosnt entirely trust Amy as he used to. In TRF he most certainly wants her away so he can go investigate the Flesh without her. Was going to drop them on earth and go alone remmeber.
2) It would explain TIA and his death, BUT with the place exploding right after, I dont think so.
3) Maybe. Knowing Moffat he'll flashback to or explain some point where the Doc thought about it *rolls eyes*
