I would assume that Ryuuken runs into Fakir and Mytho for their early morning ballet practices on the rooftop then. And general hospital interactions since Ryuuken's become Mytho's default listen to authority figure in the hospital. Which probably means he quietly pesters him sometime asking about tongue depressors and does it make people sad when they use them, Dr. Ryuuken?

Also since Azula and Fuuma share some social circles I figure she'd pick up on his health state even if they don't talk. But I think she would engage in idle chit-chat with him since he's interesting but not someone she'd consider pressing for details given his personality.
