Ms. Myers,
This letter is to give you an update on the condition of Mrs. Augusta Myers, 60 years old. She has been my regular patient for 3 years.
The advancement of her illness has been slow but progressive, and despite our best efforts, as well as varying combinations of medicine and treatment, has moved into stage 4. Though you visit often, she continues to react with confusion and often falls into fits of despondence. Unfortunately, moving her to a facility to better treat her condition is no longer an option due to her fragile mental and physical state.
At this point we are able to make the time she has left comfortable, but there is little more we can do to treat her condition. A bill will be sent to your address regarding payment for her long-term treatment.
