Grad der Behinderung is the degree or level of disability.
As far as I know, it's expressed in per cent after a medical examination. With a degree of 50% and above 50%, a person is considered "severely disabled" and gets extra benefits. Here's an overview of German disability law (in English)~
They go to both over here. There are some really strange rules regarding being carers as it can reduce some other benefits people may be on. There's also a rule here that once retired, you cannot apply for DLA (Disability Living Allowance) but your pension can be adjusted accordingly.
