I only know the answer to #5. He was born with those claws, the adamantium was put around them. If I understand your question correctly. Did you not see "Wolverine: Origins"?

So, after they got chopped off, they regrew?

Well, they're bone, I think. So I guess his super healing powers would make them regrow?
