I didn't find it a surprise that she didn't leave immediately after she was engaged, but it was never really stated based on what Kana said on why Kana left. Katara assumed her gran's reasoning for leaving would be similar to her own if she were stuck in that position based on what she knew of her gran since she didn't know her gran was ever engaged like that. For all we knew, Kana could have been kidnapped or left to help someone and ended up staying in the Southern Tribe because she felt she could do more there...I will not let this plot bunny eat me.
