I'd never thought of it that way. But I think that even the most tolerant person might respond to this silently because wanting to be a fireman or a doctor or something is just a future aspiration, whereas wanting to change your gender implies that you're unhappy with the way that you are right now. Even if a person doesn't have a problem with that, it seems sort of sad for someone to say it, not purely hopeful. At least, that's how I see it.
True, but is laughing at someone or calling them a freak an appropriate response to someone being unhappy with what they are? The passage isn't necessarily calling them the same thing, but calling attention to people's reactions, and the hypocrasy of saying "You can be anything you want to be" and not meaning it.

I actually looked at the link again after I made that comment forgetting that the person made specific allegations to reactions, so that sort of nullifies what I said. The whole people pretending to be more tolerant than they actually are idea is something I definitely agree with.
