I really enjoyed this. It's an interesting prompt and I like how you tied in Roy healing him. It fits in perfectly.

I can understand what you mean about the ending. It could read a little rushed. It's a tricky spot though. As for how to fix it, maybe there needs to be more reconciliation or at least more conversation since Sam never gets to tell his side once Dean kicks him out of the hospital room. That's just my opinion though. Maybe you could have Sam even more upset after Dean's heart attack because between the electrocution and his kidneys, he's in terrible shape. You could also put in something about Dean getting sick after being healed and Sam making sure to take care of him. I can picture Dean being surprised that Sam still cares knowing that he's 'fixed' now.

I liked the ending. I don't really enjoy long term ill Dean but this was just enough hurt and I loved the way you tied it to canon. Thanks for writing and sharing.
