I don't think Black Glove can be Bruce himself, unless he is so Bat-God he can now co-locate. How could he be doing Black Glove stuff and be drugged and crazed wandering the streets at the same time?

The Thomas Wayne, Jr. theory seems most likely to me.

I choose to believe Bat-mite is real because Bat-Mite OUGHT to be real.

Well, if Bruce himself is the black Glove, it is entirely possible he wrote down the plan in its entirety before sending Doctor Hurst to do what he is doing right now.
