And, I'm sorry, but Sam needs to stop calling Shep 'John'. She's his commanding officer and that is waaaaay too familiar for a working relationship.
Actually, I think that's always been kind of common in SG. Jack called Sam 'Sam'. Hammond called Jack 'Jack'. And Shep did work at the SGC for a while there, so he and Sam have already had some exposure to each other.
I think the first name usage is a more recent SG-1 development; Sam and Jack hardly ever used each other's first names, they were always 'Carter' and 'Sir', *lol*. The weird first name usage didn't really pop up until Mitchell appeared, and he and Sam started in on the 'Cam and Sam' show -- which always irritated me for the same reasons above, even if they were technically the same rank
