Ok, well....I am usually using my left hand (or both) for Xtra stimulation (funny since I am right-handed...)
While you use your tongue to swirl around while doing the normal sucking-up-down motion, put your hand around his shaft nearer to the base and slide it right along with your mouth...let your lips slip off the head as your hand twists up and over the head and heads back down as your mouth heads back down too...also, if you get him super-slippery, gently put the cupped palm of your hand around the head and GENTLY swirl...that seems to drive guys to the brink of insanity...
