When I create a playlist like what you're talking about, I tend to build it around a mood, theme, or story. For example, I have a playlist of romantic songs, and a play list for relaxing. Before I had an ipod, I used to create mixes on tapes, and I made one of songs that reminded me of my now husband once, and one called 2 am which was intended to be eclectic and a bit bizarre.
As far as transitions go, I tended to go with what sounded good, though as a general rule of thumb I mixed the intense music with something softer, and the "Oh God I LOVE this song" songs with some that, while I still loved I wasn't as likely to drop everything to listen/sing along with it.
But, ultimately, there are no rules. Do what sounds good to you.

Thanks guys! That helps...now I'm struggling with finding the right order, because I want to communicate and kind of progression of mood, if that makes sense, and I think I may have two sister playlists on my hands...
I tend to put a fairly eclectic mix in and then shuffle them all around so that I'm not listening to the same style/band/whatever three songs in a row. But then, I tend to like mixing Bagpipes with Swedish(?)Anarchist Punk Rock(I think that's what it was - inherited from Jonny) followed by Enya/Loreena McKinnet and then some Supertones in there for some ska/swing-ness.
Of course, I tend to make playlists with a specific purpose - my Cleaning Mixes(I have three) are mainly upbeat songs to blast when I'm cleaning house(Supertones, Superchic[k], DC Talk, etc). My Cross-stitch playlist is softer background-type music(Vienna Teng, Firefly theme, Barlow Girl, etc). Paper-writing playlist is the most eclectic because it has to be background-like but also keep me motivated on actually accomplishing stuff...
I am Crazy Anal Playlist Girl. Beware. ^-^ Which is why you don't see very many playlists coming out of me -- I have loads on iTunes, but THEY ARE NOT FINISHED AND THEREFORE NOT FIT TO BE SEEN BY HUMAN EYES. They are all carefully themed and arranged and ordered and played and replayed and replayed until they finally fit exactly right. But yes, I do try to create specific moods -- I'm working on one for autumn just now, which is predominantly acoustic and full of songs that sound gold and brown and orange and crimson (synaesthete much?) and have a little weirdness about them. Oh, well, and some of them actually mention autumn. :p
But then, my gigantic mix of songs that never fail to make me happy is messy and completely without rules or organisation. :D
See, I have been replaying this all afternoon, and this playlist thing is hard work. I am suddenly full of admiration for everyone else's perfect playlists (I have at least three in rotation at the moment).
I have a mood, or rather a sort of progression of mood in mind, but 1) it's too long, and I'm trying to figure out whether I may have two playlists on my hands instead of one, 2) it favors a handful of artists pretty heavily, and I'm trying to make it more even, and 3) as I try to listen to it I'm worried that I'm letting it all blur in my ear and that it's really not going to make sense to anyone else....
