Or it could be out of control diabetes which can lead to amputation, blindness, etc.

It's not universally fatal, with the "survival rate" being about 80-90%, but there are cases where the lupus attacks an organ, and those numbers diminish dramatically. People with lupus can work and live normal lives, but flareups of the disease can cause fatigue, sores, etc. A flareup could get progressively worse as the tour wears on.
