Agile works great in the environment with very vague commitments where the customer is eager to consume any bone you throw at him. But, if the consumer model is different and the customer expects certain features be a certain date, all this agile democracy fails and produces penalty charges and stuff.

The one-piece flow is another fad that conceptually resembles the agile ideology of working software being the ultimate measure, but companies usually fail at defining what "working" means.
