ETA: though someone died so Dean could live and I seem to remember them both being pretty upset about that. Dean was pretty pissed at Sam - even though Sam had no idea what was going on. It's such a great episode and one of the first ones to address how their actions to save each other effect others.
Hmmm…interesting to contemplate whether they'd do it again knowing someone would die (and another not have the chance of living) because of their actions.
I do think that S9 Sam would still fight tooth and nail to keep Dean from dying - just like he's done in the past. I really hope we get to see that this season. :)

Yes yes. And I think this has been one of my points in all this. It's only afterward that they know the price that was paid for Dean's life and I agree that neither of them would agree to doing that again knowing the outcome (though Sam would try an find another way, that's for sure!).
In 9.13, after Dean knew the outcome, he said he would do it again. I think that this very point is the major issue between them at the moment . Not even that Dean didn't give Sam the choice (I actually think the writers don't consider Sam's lack of choice a real issue), it's that Dean would do it again. It was at this point (straight after Dean said "I would do it again") that Sam sat down and tried to reason with Dean. He wasn't being ungrateful for what Dean did, he didn't say he wanted to die, he didn't say he wouldn't save Dean if he had the chance. Sam was arguing that Dean "doing it again" is the "problem

Yes, they wouldn't have agreed to it if they'd known the outcome, and they would've looked for another way to save Dean.

I'm not sure what you mean... cause Sam saved Dean's life many times in S4, and also, the reason he was so messed up that season is because he wanted to save Dean from hell (and he was supposed to, but the writers changed it so Cas did, instead), and he did try to sell his soul to get Dean out of there, but no demon would deal, so instead he focused on getting revenge on Lilith (who held Dean's contract in hell) and that took him on a bad path with Ruby.
I blame the writers for that, cause Sam was supposed to save Dean from hell, and then they changed it. I agree, it's very disappointing.
Sam saved Dean in On the Head of a Pin, when he killed Alastair. He also saved him from Alastair in Death take a Holiday. There may be more, that's all I can think of at the moment.
