I liked how cruel they had Dean be to Sam (I mean, I didn't like it but I was glad they went there). I really hope they follow through on that throughout the season and don't just have Sam brush it off as 'oh it wasn't you'.
The part I hope they don't brush off is the fact that Dean was ready to kill Sam when he was so close to human that he could walk out of a devil's trap.
I think it would have been bolder, and better, to have the whole thing just in the bunker - to really ramp up the claustrophobia. I was frustrated by Cas showing up miraculously to save the day (it would have been much more dramatic to have had Sam have to battle through solo). On the other hand, after some consideration, I do like the ambiguity we're left with about whether Sam would actually have let Dean kill him (my feeling is yes).
Yes, I would have loved to see Sam do it completely on his own (and absolutely, not waste time on other characters). But the ambiguity was really good.

With Cas and Hannah I ticked both "they're growing on me" and "they bore me to tears" - the first applies to Hannah, the second to Cas.
Regarding the question of what Sam would have done if Cas hadn't appeared when he did I'm missing the option: No, Sam wouldn't have killed Dean, but Dean wouldn't have killed Sam either, because Dean was mostly concerned with goading Sam into killing him and making poor Sam miserable forever. :)
Well, that's an interesting possibility that I hadn't considered. Personally, I think Dean did mean to kill Sam, because of how he reacted when Sam dropped the knife from his throat. But if all of that was an attempt to scare Sam into killing him? Hmmm. I need to ponder this.
