If you can, find an audiologist who specializes in APD, or at least knows about it & can test for it.
I recently got back from seeing my audiolgist who specializes in APD, and I now have a pair of hearing aids. BEST thing EVER! They are extremely helpful for me & my type of APD. Not all audiologists are willing to do hearing aids for APD, it's kind of a new thing. And it doesn't help everyone with APD, they don't quite know why that is yet.
And your edit 2: That's normal for me, it takes a GREAT deal of my concentration to understand/hear people. If I'm extremely occupied with something else, I likely won't hear them at all.

Yeah definitely sounds like APD.
I also have hearing aids and they really are fab.
The only thing I don't get is the shouting bit, but it is distracting if someone else is shouting and for me can even be painful.
