Riddle's Diary

I'm in the process of planning out a fanfic, and I wasn't sure about the details of the diary as a Horcrux. Is it canon that Voldemort used the murder of Moaning Myrtle to turn the diary into a Horcrux, or is that just an assumption I've made in my head? I've checked the HP Lexicon and came up empty. Any help would be appreciated.

He couldn't have killed Myrtle to make the Horcrux because he didn't know how to make them yet when she died. Myrtle was killed before Riddle murdered his parents and stole the ring which he's wearing in Slughorn's memory of the Horcrux explanation.

Well, after she said that, I went and looked on the Lexicon again, and came up with this:

This says that Myrtle was killed AFTER Voldemort murdered his parents and stole the ring, which contradicts vejgurl's comment ( ... )

I was under the impression that he was in his seventh year when he had the conversation with Slughorn.

However, we know that Riddle eventually became Head Boy. If he was wearing a prefect's badge, it could not have been his seventh year.
