Stormwind is basically an idealized medieval fantasy: we have castles and kings and indoor conveniences without all the hygienic nightmares. You could safely assume that homes in SW have bathrooms and toilets.
I don't play Horde, so I can't really answer for Orgrimmar. I imagine that they look more primitive than Alliance towns, but I'll ask a Horde friend about this.
