You have to determine your general philosophy about baby stuff. Some people say that it all falls into three general categories: things you buy in advance and use, things you buy in advance and find useless, and things you end up needing to buy later.
I figured this is stuff we'll be buying in advance. I'm sure we'll need more stuff later and I'm pretty sure this won't be useless. If you have specific stories on why soem item turned out to be useful OR useless please share. Also, as bloguser said... khm.. you know.. if there's stuff you won't need by then, let us know.
You can use your favorite backpack as a diaper bag. If you buy a diaper bag, it will probably come with a portable pad. I didn't like mine, but found nice disposable ones by Huggies. I get multiple uses out of each one

diaper bag I would say you don't need it -- a regular backpack works just as well. If you do want it, we have a new one (with tags), that you are welcome to. It's not the backpack style though.
home changing pad Do you have a changing table? If you are going to buy one, it'll come with a pad. Don't buy pad covers, we've been using pillow cases, and they work great -- nice material, and much cheaper than $19 per changing pad cover.
portable changing pad Your OB/Hospital will have a free one to give to you (sign up for various "free baby stuff" mailings, and you'll get coupons for free ones)
diaper pail odor free That's useful.
changing table/ dresser w/drawers See above about it coming with the changing pad.
carseat You are welcome to ours. Graco saferide. 2 bases.

Hmm... First of all, you'll still get useless stuff, but that's ok. If you put too many things on your registry that you only mildly want, you may not get the ones you are hoping to get. By the way, angerona mentioned Target--it also has a registry.

We didn't find our baby monitor useful, but that says more about the layout of our house than about baby monitors.
We used receiving blankets for swaddling with pretty good success. We bought our favorite type for Mona already :)
