Maybe there's a deleted scene where the camera pans up and the creepy ghost of John Winchester is perched on a lamppost at the end of Dean's line of sight.

This ep is the first one in five seasons to actually make me gag and then howl with laughter...and then gag (fryer)etc.

There was much goodness in that ep but it did bring up several questions. Most of which you covered.

Biggest one being: Who the heck does Dean think he is talking to?

I wonder sometimes if they are going to pull a "Dogma" type thing and god is trapped in a coma somewhere after playing Skeeball in New Jersey.
