Don't know if it's any comfort, but I also have pretty much all of those in spades -- I've learned to manage them, but they tend to lurk under the surface. I'm still terrible at small talk, and Kate pretty much despairs of how avoidant I get about making phone calls. And the "not wanting to bother people" one is terrible: I force myself past it sometimes, because it can be crippling in the sort of leadership roles I put myself into, but it is harder than hard.
I don't think of them as social anxiety per se, but this particular syndrome does seem to be somewhat common in geekery...
