think it's necessary to get permission forms from people when writing something that is Inspired by True Events but not Based on True Events?

yes, there is a difference.

Inspired By=liberties were taken
Based On=strictly based on actual events, no liberties.

thoughts?

Inspired by -- I wouldn't think you'd need permission forms because, as you described it, it's taking liberties and it's not the actual thing persay.

depends on who the person is (whom the events happened to), and how far from the truth it deviates.

but most people would be flattered if events in their life inspired someone so it may not matter at all. unless of course its a bad thing..
