I started to read a series ages ago, but can't find it now.  Harry is sentenced to Azkaban after killing Voldemort as Dumbledore feels he is too powerful.  Severus and Draco slip him a port key to help him escape, which he finds after some months and uses.  Severus, Draco and Harry form a new order (can't remember what called) to fight new
