As far as Onslaught Reborn is concerned, it's supposed to take place in a new alternate reality as opposed to on the original Reborn Earth, which has seen a lot of changes and is currently orbiting our sun as Counter-Earth. Of course, this hasn't been said in the series yet but was said in an interviw with Loeb that I read. Not that removing the continuity problems really makes it any better.
