Here’s the thing no one understands about her choice to be a sober companion: the reason she’s so good is because she knows what being an addict feels like.
Here’s the thing about addiction: drugs aren't defined by simple chemistry. Drugs don’t need to be tangible, physical for them to create an addiction.
These two lines ring so true to me, with how Joan has reacted to everything. It's obvious she takes life straight-on, with both hands. But even that can be an addiction, and I love the idea that she enjoyed playing God (who doesn't, in the end).

yeah, i think people sometimes tend to forget that Watson was *more* than just Holmes' companion! In this case, I think in order to be good at her job, she must have somehow gotten first-hand experience of what an addiction feels like, and (imo) becoming addicted to her work sort of explains why she backtracked when her patient died.
(or, it could just be me grappling blindly for some Watson backstory.)
