That is entirely possible. When my wife worked for an ENT, they did extend special privileges for those in the industry. Lynn may have gotten those by playing her "I am dentist's wife" card.
They do fees now for cancellations, but there was a long stretch when they did not do that. As for the cancelled appointment in an hour, that is incredibly fortunate. These days, I am amazed at how long the wait time is until the next appointment. My dental hygienist books her appointments a year in advance.
I thought the "joke" there was either that whatever cafe the old fart goes to is too far away from the pharmacy or he expects to be made to wait for a lot longer than 15 minutes. Or Lynn is trying to make some point about things being better in the old days when drugstores had little restaurants in them.
