It feels like he still cares for Sherlock but doesn't understand that the feeling is reciprocated. Though that would be partly Sherlocks fault too.
Pretty much this. There's also the distancing aspect of finding out Sherlock's the species John is duty-bound to kill. Sherlock ought to be a monster, so of course he's simply been using John - except John is simultaneously certain it was more than that. Generally confusing.

Gads the tension between these two. Still caring about each other, but John doesn't know what to think about what Sherlock did and Sherlock is on such uncertain (but well-deserved) footing.
