Don't moisturise your hands. The oils from the moisturiser prevent proper sticking of the polish to the nail. Take a cotton ball with nail polish remover and swipe all your nails before you apply nail polish. Wear a base coat. If there's bubbling, you're either putting on too thick of a layer or not waiting for it to dry completely before applying colour. If your base coat is goopy, thin it out with nail polish thinner. Make sure you apply thin layers. Same with your coloured polish. Thin layers, let dry completely in between. Use a good top coat, too.

Wear gloves when ever doing housework or dishes. Don't let cleaners or water get on your nails if at all possible. Also, make sure your nails are free of oils or moisturizer before painting. Clean them with remover before applying base coat. Make sure each coat is thin and dry before applying the next coat. I like to put another coat of top coat on after a few days to keep the manicure fresh. This helps it last an extra few days. Hope this helps.

Don't use moisturizer/lotion/oils before you polish, and if you're using a moisturizing polish remover before you repolish, make sure you clean your nail beds with alcohol before applying new polish, to get rid of oils that may keep polish from adhering. Also, it sounds like your old base coat might have been old and thick and not drying properly; one like Stickey Base Coat by CND might work. And I second the previous advice about wearing rubber gloves to do dishes and housework involving cleansers or solvents, it really helps.

Always swipe your nails with polish remover or alcohol before applying your polish (or base coat). Your nails should never have any lotion or oils on them prior to polishing.

You might just need to try a better base coat, because it will definitely help the polish adhere to your nails and not peel or chip off. I use China Glaze Strong Adhesive base coat and my manicure lasts a week easily with no chipping or peeling. (I also use CG polish almost exclusively)

We don't have China Glaze here..boo.
I feel like reapply top coat after a couple days (which I totally do that all the time) stops it from chipping, but not peeling..which is frustrating as all heck. I use OPI Rapid Dry Top Coat.
And, I have/had (might've chucked it) some Revlon base coat...maybe I will try a different brand.
