before flying, check on the cane. Chance they won't let it on the flight, so it may need to get packed (couldn't tell if it was being listed as a to-pack-item or a carry-on item).
They've never refused to let me bring it on a plane. It's one of those telescoping jobbies from a medical supply house. It'll probably go in packed luggage on the outbound trip, but I'll need it by the time I'm coming home.
For cane: would one of those telescopic walking sticks work for you? It'll still be better off in your main luggage, but it might be handier elsewhere.
T-shirts: I don't know about Malta, but I've always found Spain to have a really nice selection of not-too-naff T-shirts in all kinds of colours, so rather than buy them at home, I tend to buy them on holiday and then be reminded of that holiday.
If you take walking boots, will you need special socks?
Batteries you can probably buy at location, too - if you take rechargables, make sure your charger works with the voltage over here.
Travel money - always have enough for the first couple of days; and stock up on something to eat/nibble, even though you can't take anything to drink.
* sleeping mask? I once went on holidays and found the curtains were so flimsy I was waking up at daylight like the birds. Had to go buy a sleeping mask, 'cause waking up with the dawn is not ever on my to-do list. * Photocopy of all documentation in your luggage. * Copy of all documentation with someone trustworthy at home. * beach bag for hat, towel, books and sunscreen (and my best wishes for a wonderful, relaxing holiday)

I have discovered that the fancier AA batteries intended for cameras do in fact last a lot longer than the regular AA batteries.
