Two things I don't understand, though:

"He wasn’t sure whether the ghost somehow understood what he’d said or not, but suddenly it jumped to its feet and stood right in front of him. Which was where the coffee table was, proving to Ianto once and for all, as if proof were still needed, that the man before him was indeed a ghost, since its legs and the lower part of its coat vanished through the table top and reappeared underneath."

Okay, let's see if I can explain my thought processes... Jack sees the real world, while Ianto sees a ghost version. Everything Ianto sees is real to him, it's the world he inhabits, along with a few other 'ghosts' who can walk through Jack because to them, he doesn't exist.
Then at Ianto's 'real' apartment, that's where Jack spends his nights, so Ianto is actually the one haunting Jack there. The bed is still there, so they can both lie on it, and some other furniture is there, but other things have been moved, or taken by Rhi - it's been a year since Ianto's death after all. Ianto still remembers the coffee table. It's there for him, but not for Jack, so Jack appears to be standing in it. Even the book Ianto checks out of the library is only 'real' to him, but Jack vcan see it when Ianto holds it because it becomes an extension of him.
Does that make sense?

To be fair, Ianto thought it was ghost Jack for a long time too ;)
