Steve suffering in silence is a favourite troupe of mine too. Have you read Routine
or Harlem Shake
or Pot Calling Kettle

In all cases the pain is internal/psychological and the main character is Steve.

Hah, glad I guessed well. Actually, was looking for something else and came across the link to Nowhere to Hide which is also along the same lines with suffering in silence, but does actually have a bit of 'serum having changed Steve sexually' (not a major theme, but it's there). Couldn't find it yesterday (figures), but if you haven't read it I highly recommend it too.
