I love every word of this. You've captured the ache of the years on him, and how much he's had to do. I love that part of this is fear of the Valeyard even as he recognizes that particular timeline is probably not going to come to pass now, but the potential is still inside him. I love that he has known enough immortals to know what becomes of them, and that he has known enough mortals yet still feels the loss of each one when he can't have a moment back with any of them. This is perfect characterization for the person he was when he reached Twice Upon a Time, and I'd like to think this is what she'll carry forward as Thirteen.

I wondered after I wrote it whether I should include the bit about the Valeyard. After all, new fans may have no idea who he even is, and older ones usually seem happy enough to forget about him. :) But he's such a handy, concrete symbol of the Doctor's capacity to be a worse person than he is.

The fact that immortality is almost always a bad thing on Who is something that's been on my mind ever since he exceeded the regeneration limit with, apparently, no new limit in sight. And the pain of constantly losing his more mortal friends is clearly something that's been on his mind for a while...
