Cleveland Browns (4-8) at
Pittsburgh Steelers (9-3) -13.5

This means Cleveland currently has 4 wins and 8 losses. Pittsburgh has 9 wins and 3 losses. I.e., Pittsburgh is much better than Cleveland is. The important part is the spread at the end (-13.5). Because the spread is negative in this case, it's favouring the first team (Cleveland).

If you choose Cleveland and Cleveland loses by more than 13.5 points, you lose.
If you choose Cleveland and Cleveland loses, but by less than 13.5 points, you win.
If you choose Cleveland and Cleveland wins, you win.

In other words, if you choose Cleveland, they have to win or lose by less than 13.5. If you choose Pittsburgh, they have to win by more than 13.5 points.
