Loved this: “I didn’t eat when you were dead,” he says. because it just shows his insecurities so well, and that whole part there shows how he's not really sure what's up with his brother.
And this “What, the thing with the-” “You got arrested for pissing on the side of a police car.” I think is the perfect example of how you made the original more (or better, even). It's so much stronger here, which just shows what an amazing writer you are.
I also loved this: It feels like maybe the experience should be familiar, but it’s not. Dean’s arm is cool and unbranded under Sam’s palm. . And this Somehow, they fit, even facing in opposite directions, just feels so season 4 to me.

Wow. I really enjoyed this. I love the way Dean is repeatedly put out by tiny (false) indicators that Sam doesn't appear to have been pining, and the way Sam keeps needing the reassurance that Dean is here. It works well in s4.
The sudden frantic hotness is lovely, and then this: After a minute, he says, “Shower?” because coming in your pants gets less and less awesome as the minutes tick by made me gleeful because it is such a kink of mine, but then the reality and the humour kick in...
One teeny thing: it should be "settles in vertebra by vertebra".
