Send the packet to her home address if you have it. I did this with a retired LOR writer. I printed out my old papers from her class, the LOR form, my statement of purpose, and the emails I had been sending her for the past few months that she hadn't responded to since she just retired. She responded via email once she received the packet and wrote me the letter.
Email clearly isn't working, so I would try to call her at the office, call her at home (whitepages.com). Also, try calling the department admin (if there is one) to see if she has a better way to get a hold of her. Also, I like the suggestion to send something to her home address.
I have a similar problem as well, except my deadline has passed. My suggestion is call up the admin office and see if they will call her for you to remind her, or give you her contact number.
