Hee, I must love this one, second time I've read it. I like that Merlin's point of view is not used, so it's a bit mysterious, how long did he have feelings for Arthur, was that motivation for the piercing, or was it just something he would have done for himself, regardless of Arthur's reaction. I like how it alerts Arthur to the sensuality of his best friend, and changes things, which makes relating awkward (isn't it always when you get a crush on someone).
