The Loathly Lady story isn't exclusive to Chaucer, and there's nothing in this one that's particularly drawn from Chaucer's version.

Did she appear in any other Arthurian stories? I had the impression Chaucer's was the only one.

Chaucer's version isn't even the only one from his time period! It's in Gower's Confessio Amantis as well, and is apparently a fairly common folk tale (not always attached to Arthur's court). It seems like the name Dame Ragnell comes from a 15th-century romance (The Wedding of Sir Gawain and Dame Ragnell).
